Integrace Bayleigh Chase, a forward-thinking life plan community based in Easton, has introduced a new rehabilitation program for individuals recovering from stroke. Designed to provide a full continuum of care for stroke patients, the program offers a progression from short-term rehabilitation to outpatient therapy, with ongoing follow-up from Bayleigh Chase’s neurocognitive support team.
The Stroke Rehabilitation Program at Integrace Bayleigh Chase is guided by a specialized interdisciplinary team led by neurologist Terry Detrich, M.D. and Medical Director Paul Reinbold, M.D., and supported by Neurological Nurse Practitioner Christina Ball along with nurses, social workers and physical, occupational and speech therapists.
Guests in the Stroke Rehabilitation Program at Integrace Bayleigh Chase benefit from:
– A minimum of three hours of daily physical, occupational and speech therapy with specialists, along with additional hours with the restorative team for Medicare and/or Medicaid advantage plan patients;
– Access to the expertise and services of the Bratton Neurocognitive Clinic to address any memory problems;
– Individual and group therapy sessions offering the opportunity to practice skills and share experiences with others as desired;
– A transitional care nurse who carefully coordinates the individual’s stay, discharge plans and follow up care;
– Education to help family members and caregivers understand potential changes in their loved one’s behavior, while addressing risk reduction strategies and other health conditions that may be affecting quality of life
